Client Consent for Use WhatsApp Communication

Purpose:

You are being asked to sign this form to confirm your understanding of the potential risks of using Whatsapp to communicate with your agency staff and to give your informed consent to do so. The SLCN does not currently recommend usage of Whatsapp, but understands this may be the best way to communicate for families at this time. Please review the following information before proceeding.

What to Know:

• Third Party apps, such as Whatsapp, are not fully secure for client communication

• Although WhatsApp uses end-to-end encryption, it is not considered HIPAA-compliant, and your personal health information may be at risk

We will limit use of Whataspp to non-sensitive communication, such as:

• Appointment reminders or rescheduling

• Arrival updates (ex: I’m here")

• Sharing links to public resources (ex: a mindfulness video or link to referral)

• We will not use third-party apps or texting to discuss your personal information, mental/emotional health, assessments, or emergencies

Risks of Using WhatsApp: By using Whatsapp, you accept the following risks:

• Messages could be seen by others if your device is lost, stolen, or accessed by someone else

• You may receive notifications on your screen that others could see

Your Rights

• You are not required to use WhatsApp

• You can request to use a more secure method of communication (Like phone call or in person)

• You can change your mind at any time and withdraw this consent

• SLCN can also decide to withdrawal usage of Whatsapp at anytime
